Reading Survey

(Grades K-2)

Please complete this survey with all students in kindergarten through 2nd grade. The survey can be administered to students individually, or in small groups. Read each question out loud to students. Students should use the smiley face likert scale to rate their feelings about reading.

🙂 = Yes    🙁 = No    😐 = I’m not sure
The student may complete the survey or an adult may complete the survey for the student.
Surveys should be completed for all students no later than February 17, 2023.
